Residents in nursing homes across Ealing are set to benefit from a new GP service that will offer care 12 hours a day, 365 days a year.
The service has been introduced by Ealing Clinical Commissioning Group (CCG), which is responsible for planning and designing many health services across the borough, in response to the increasing complexity and needs of nursing home residents.
Argyle Road Surgery will provide the service to over 1000 residents across 22 nursing homes. The surgery will recruit staff to deliver the service who will provide frequent reviews and follow-up of patients; rapid response to requests for urgent visits, and a specific focus on improving end of life care.
Dedicated medicines management support to those patients who are on multiple medications will also be provided.
Dr Shanker Vijay, the project’s clinical lead, said: “The service has been developed with input from several key stakeholders, including patient representatives, carers, nursing home staff and local clinicians. The aim was to design a service that would meet the needs of all those involved in residents’ care.
“Our sincere hope is that providing this enhanced primary care service for nursing home residents will improve the quality of their care and reduce the number of avoidable emergency admissions to hospital.”
Dr Mohini Parmar, Chair of Ealing CCG, added: “This is an innovative service and, to the best of our knowledge, we are one of the first CCGs to design a dedicated service specifically for nursing home residents. We are committed to monitoring this service to assess the impact it has on our patients.”
Argyle Road Surgery will work with the nursing homes to provide information about the service to residents, carers or family, as appropriate, so that informed patient choice is maintained in the registration process.
